| 3. Determine whether standing orders should be continued, modified, or discontinued at time of transfer: | 4. Complete step 3 for PTA medications. |
| 5. Add new orders for the floor: | 6. Sign and hold the orders. The “Sign & Hold” option allows you to complete the reconciliation prior to the patient’s physical transfer – the changes you make won’t take effect until the patient arrives on the floor and the nurse releases these orders. |
