When transferring a patient between levels of care (e.g. from the ICU to the wards), an order reconciliation should be completed. The following steps are essential to ensuring optimal care transitions.

3. Determine whether standing orders should be continued, modified, or discontinued at time of transfer: | 4. Complete step 3 for PTA medications. |
5. Add new orders for the floor: | 6. Sign and hold the orders. The “Sign & Hold” option allows you to complete the reconciliation prior to the patient’s physical transfer – the changes you make won’t take effect until the patient arrives on the floor and the nurse releases these orders. |
